Paragraphs 25-26, 29-30 indicate:

  1. planned dates of entry and exit;
  2. last day of validity of the previous visa (if any);
  3. day of border crossing upon arrival (including by air);
  4. date of departure from the airport or border crossing on the day of departure.

Paragraph 27 obliges the applicant to provide information about the fingerprinting procedure using the words “yes/no”. If the country that is the final destination of the trip is located outside the Schengen zone (clause 28), you should indicate the details of the previously obtained visa to visit the destination country.

The details of the persons who issued an invitation to visit (clause 31) are filled in in accordance with their identity cards. If there is no invitation, the data of all hotels where accommodation is planned is entered in the paragraph. If the trip is of a business nature (clause 32), the details of the company that issued the invitation to the applicant are entered in the item field.

Clause 33 is financial in nature. In it, the person filling out the form must indicate the origin of the funds that will be used to pay expenses for travel, stay and accommodation in the countries of the Schengen area. For minor citizens of the Russian Federation, the sponsor is a parent (parents); for non-working citizens, the sponsor is the inviter or another person.

Points 34 and 35 are filled out by applicants who arrive in a Schengen country to visit close relatives who are citizens of EU countries. The last two points of the questionnaire (36 and 37) are for informational purposes only:

  1. place and date of application submission;
  2. personal signature of the applicant.

If the application is submitted by a minor citizen of the Russian Federation who is under 14 years of age, one of his parents must sign. If the applicant is 14-18 years old, the document, in addition to the parent, must also be signed by him.
